    Semaglutide Weight Loss Therapy Program

Cost:

$150 (4 weeks) for 0.25mg dose

$175 (4 weeks) for 0.5mg dose

$200 (4 weeks) for 1 mg dose

$300 (4 weeks) for 2mg dose

Initial Consultation (Regular office visit billed to insurance, if self-pay visit is $100 plus cost of labs ($30) if needed)

· You will establish your weight loss goals

· Patient should bring lab results (CBC, CMP and TSH) from PCP if done within the last year. Otherwise, labs will drawn at the first visit (do not need to be fasting).

· Education will be provided on Semaglutide and what to expect

· Instructions will be given on administration of the Semaglutide

· The first month of Semaglutide therapy will be a dose of:

o 0.25mg weekly 

o Patient should administer 1 prefilled syringe weekly

o 4 prefilled syringes will be dispensed

1-month Follow-up Visit (Included in monthly price)

· We will make sure you are tolerating the therapy and achieving weight loss goals

· Discuss titrating the dose of Semaglutide

· 2nd month of Semaglutide therapy will be a dose of:

o 0.5mg weekly

o Patient should administer 1 prefilled syringe weekly

o 4 prefilled syringes will be dispensed

2-month Follow-up Visit (Included in monthly price)

· We will make sure you are tolerating the therapy and achieving weight loss goals

· Discuss titrating the dose of Semaglutide

· An ongoing follow up schedule will be determined

· 3rd month of Semaglutide therapy will be a dose of:

o 1mg weekly

o Patient should administer 1 prefilled syringe weekly

o 4 prefilled syringes will be dispensed

*If more than 2 weeks of injections are missed the program will start over.
